Algebra ECA - May 21-May 24

These tests  help determine grade level proficiency are are used for placement.

Attendance is extremely important.  Also, please encourage a good night sleep (8 hours) and breakfast.

Congrats to our Tri-Creek Education Foundation Grant Winners
  
All incoming 6th graders are required to have a current physical on file with the office prior to the start of school.  They will also need the vaccinations listed below.  The physical form is the same as the IHSAA physical form.
 
The State of Indiana is requiring all 6th through 12th graders to have the following shots when they enter school:
  • A second VARICELLA VACCINE (Unless proof of disease is on file.) 
  • 1 Tdap VACCINE 
  • 1 MCV4 VACCINE
Supply lists are located to the left under links.  With the 1:1 computing initiative, all students are encouraged to bring ear buds or headphones that are comfortable to him or her.

  • REMINDER!!!
    Friday May 31st is the last student day
    All medications must be picked up by the last student day of school;  Any medications that are left after the last student day will be destroyed.  STUDENTS ARE NOT ALLOWED TO TAKE MEDICATIONS HOME ON THE BUS.  ONLY THOSE STUDENTS CARRYING INHALERS, DIABETIC SUPPLIES OR EPI-PENS MAY CARRY THEIR SUPPLIES/MEDICATIONS HOME
    New immunizations required for school year 2013/2014 per the Indiana State Department of Health
    * 2 doses of varicella(chicken pox) vaccine or evidence of immunity will be required for all students in grades k-12
    * 1 dose of meningicoccal conjugate vaccine (MCV4) also known as Meningitis
    * 1 Tdap vaccine
    Sports physicals given on June 6th at the High School  Watch the website for new information pertaining to sports physicals.

    In an effort to provide a safe, anonymous way to report bullying, we have created a link to a form.  Please use this form to report bullying at LMS. We are continually looking for ways to provide students, parents, and staff convenient ways to report unsafe actions at LMS.  Please remember bullying is defined as using an imbalance of power to repeatedly, purposefully, harm someone.  Please contact us with any questions.
